WWII German dog tag "sample" with original chain and description tag.

I found this at a little shop 2 days ago, is anyone familiar with the document/publication referenced on the tag?

Never seen such a label before, if it is authentic, this tag is a production sample for a dogtag for AFV crewmembers.

The shop owners' younger brother had been in Europe many years ago, and occasionally sent her boxes filled with a variety of items such as this, plus original propaganda posters, Nazi recruiting posters and flyers, wooden ammo crates, etc. to sell at her antique shop. Judging from several other WWII vintage German items in the shop, I have no reason to believe this is not a genuine article. All she knew was that it was from wartime Germany, nothing else. She's getting down to the last few items that he sent from the sound of it.
